It's very clear that baby will "smoke" together with his mom, through the umbilical cord, and after born, when nursing. Excelent work, very shocking and real...

Since the other comments cover all of my opinion, I go directly to a nitpick: I think you should modify the smoke a bit, so 2 of them look different (we can't see smokes with the same shape twice, right?). You got one of the highest votes I've placed here , best of luck to you!
