Remove the white that was between each color chip by going to: Select, color range and clicking on the white, and then hit delete.

Select connected chips and moved them to a new document. Then go to: filter, liquefy, and smudge to give them a liquid look.

After this is done to all of the paint chips, move them together to create a solid image.

Merge all of the layers together and use the liquefy filter once again to blend the edges

Open up the image of the hand and duplicated it. Make a duplicate copy and make it black and white by going to: image, adjustments, desaturate. Then go to: filter, blur, gaussian blur at a value of 1.5. This blur is an important step.
Save as a PDF and close it.
Move the image of the liquefied paint chips over on top of the original image of the hand. Set the opacity of the paint layer to 50%. The go to: Filter, distort, displace. In the dialogue box type in 20 for horizontal and 20 for vertical. Leave all other settings alone. Hit OK. It will then ask what image is to be used as a displacement map. Select the copy image that was saved as a PSD
