Started by masking the knife and putting it where I wanted it. Used a nice hard edge on the mask where it went into the "victim"

Added a crack with the pen tool using a color selected from the "victim" them burnt it a bit until it looked like I wanted. Had to liquify it a bit to make it match up with the knife edge.

The blood is on about 5 different layers so I could move each peice. A rough draw followed by some liquify and bevel to give it the feel I liked. Turned off to Global Light in the bevel so each part looked right. Wanted to show the last trickles of blood, thus the wide stains where the blood flowed freely following the initial vicious attack by an unknown assailant.

Each eye is 3 parts. The pupil is just a semi fuzzy dot. The eyeball is a white circle beveled to look round. The eye socket is behind them all and is a circle on the avacado itself with a large pillow emboss to the outside. Positioned the pupils to ensure a good "guilty" look.
Each part is on a different layer so I could properly position them.

Eyebrows. The heart of facial expression. Used the "victims" stem with some liquify and shadow. Whated them to stand off the "face" a bit for a more "cartoony" look.

The mouth was the hardest. Copied a circle from the skin and cut a hole in the middle of it. Then used liquify again to get it the shape I wanted. Gave it a large bevel / emboss and filled the hole with black. After about 1 million tries I got it the way I wanted.
Finally gave the knife a shadow similar to the ones on the source.

Simple red dots (for contrast) and some liquify gave me the shape for the beads of sweat. Then I selected that layer and copied that selection from the source image. A little bevel and some lighting work and Voila! Sweat

Forgot to add this so some of this SBS is out of order. Sorry.
The pool of blood began as a simple red shape (1) Had to use some liquify and other simple tools to make it fit snuggly around the base of the avacado. Then I gave it a little burn to make the shadow and give it some depth (2). A bevel with the light source brought almost directly above gave me the final product (3)
You didn't explain how you made the blood poo, which is a major element...
