I made a selection of the hand, covered it with the grass and set the layer style to overlay and played a bit with hue/saturation to get the green, I wanted.

Adding the horse, tree, girl, path, sunflowers, bird, shadows and some dodge and burn here and there.
step 3 of 3
To get the dreamy effect, I merged all layers, copied this layer, used gaussian blur with the amount of 4,5, set the layer style to overlay and the opacity to 65%
