Close cut image

Place close cut image over face and use displacement filter.

Close cut after displacement filter has been applied.

Now multiply displacement layer and background layer. Use opacity to find the sweet spot.

Duplicate layer and use levels to give you this effect.

Use a layer mask and remove the middle of the face and fine tune your image.

Play with levels and hue/saturation to find the color and contrast you like.
