I duplicated the layer and then increased the size of the canvas. I then flipped the duplicate layer horizontally and placed them side by side.

I then copy pasted the cetre part on a new layer and using liquefy, made the transition of the images more natural.

Working with the doll on the left, I changed his eyes so that he was looking more towards the other doll and changed the mouth to make him look sad.

I then cut out the girl's face and placed it on the doll on the right. I was going for a comical out of proportions look to keep the doll feel.

I then cut out the feet (done on separate layers) and transformed them for size and positioning.

I did the same with the hand with the cigarette.

Next was to cut out the hand with the beer and place that in the image.

I created a new layer and then cloned out the unwanted background. This part was quite tedious.

Next step was to try and match all the skin tones as best as possible since they were from 3 different people.

I then copy/pasted the cap onto the girl's head. (I just noticed that I had the color adjustment for the girl's face off for this step)

I redid the collar and started adding some shadowing to the girl.

Next step was to start adding some shadowing to the couch.

I added the bowl of popcorn and all the shadows required.

I then added some smoke to the cigarette (and finally remembered to turn the face color adjustment back on).

Added more smoke to the upper part of the image.

Added some mist to the smoke using Render>Clouds.

Sharpened the image and made some color adjustments to finish.
