Started by using the clone tool to remove the doll's eyes and mouth

Using the clone tool and the heal tool, used the blank portion of the dolls face to create a clean area around the eyes,

Pasted just the eye area onto the doll and adjusted it so it fit well where the original eyes were. As I used the free transform tool to rotate and size my eyes, this layer's opacity was down so I could easily see where they would be positioned

Going around with the paintbrush tool I cleaned away the dirty portions of the pasted eyes, and blended the painted "clean" sections of the eyes into the dolls head

I repeated the above steps from the eyes again with the mouth, and cleaned up to finish the face

I selected only the hair, made a new document and turned to black and white. after doing that I adjusted the levels, hue, and saturation until it was a decent shade of yellowish blond

I then pasted the new colored hair back into the document and adjusted the opacity till it resembled a more natural look

I cut out the arms and dress from the second doll image and selected just the arms, fine tuning the color to match the existing doll. I then used the warp tool to line the arms up with the dolls existing arms.

I sized and blurred the background

Finally I removed the unneeded portions of the frame picture and adjusted it to the correct size of my image
