I took this picture of a cigarette. Macro setting for clarity. Then I layered it and sized it to fit the squirrel's paw. Then used the elliptical marquee tool to make a speech bubble with the eraser, along with the tail. Then used the vertical type tool with Eras Demi ITC as the font.
Pretty simple really.
