i masked out parts from the source to use...

cut out plate from source, warped, an dlayered over head

i copied the face, and used liquify to warp the eye, then erased the rest, or so i thought, cuz i can see a big piece at the bottom i forgot, but it blends so no biggie

i copied a section of the skin on the head, warped and blended the edges on a lowered opacity

i used a piece of flesh from an image from "Package - Slaughter - 9" by #resurgere at deviantart.com to make the skin look fleshy

used another piece from the pack to make the torn area look fleshy as well...

used another piece of flesh to make the eye area a little redder, blended on lowered opacity, again, forgot to remove all of the excess....

used merged and warped pieces of source to create this thing sticking through his nose....

i copied the end sections of the previous layer, and arranged them so they stuck out from the other side of the nose...

other piece....i darkened the areas underneath everthing sticking into the skin using a burn on the face layer

used another piece of flesh for the nail area, blended on lowered opacity and burned to make drip

used a black fill layer with a red omni blended on color dodge at lowered opacity to add some color....
