Create A black canvas and add a white boarder ensure the boarder is slightly larger on the bottom. Sizes vary depending on image.

Delete the black in the centre of the polariod surround. Ass a bevel emboss layer style (all settings keeped as default with the exception of the 'soften' slider this was increased to maximum)
At this stage also add a pattern overly layer style. I chose white texture which can be loaded as a default in photoshop

Copy the polariod surround and paste into source file. Use free transform to rotate surround and position accordingly

Copy the polariod surround layer several times (5 times in my case to create 6 polariods)
Use free transform on each of the copy polariod surround layers and position and rotate until desired layout achieved.

Copy the contents of the background layer (source image)
Then create a new blank layer at the bottom of the stack just above the background layer and fill with white.

Select the first polariod surround layer. Use the magic wand to make a selection inside the polariod and use the paste into command. This will paste the portion of the source image that you copy in the earlier step inside the polariod surround

Repeat step 7 for each of the remaining polariod surround layers.
Be sure to use the paste into command not the standard paste command.

Add a layer style to each of the polariod surround layers. I used an angle of 34 degrees, distance 11, spread 0, size 21
