step 1 of 5
Cut out a section from the top of the lips and used the bloat tool to create a 'sucker' for the leech.

Filled two concentric circles on the 'sucker', the outer white and the inner mid-red, and then used the smudge tool to 'pull' teeth(!). Added a red splodge at the centre to represent extra bloodiness!
step 3 of 5
Filled the background black and overlaid green text of home grown scam e-mail.
step 4 of 5
Applied a clothy texture in an attempt to simulate a pixelish effect(!) and reduced opacity of texture to ~20%.
step 5 of 5
Overlaid (in overlay blend mode) a circular white -> black gradient and reduced opacity to 50%, to simulate screen glare - et voila!
