First I took the wood closeup, and used the warp tool and a layer mask to make the right shape.

I did the exact same to make the mouse buttons and a scroll button.

Then I took the pen tool, made shape of a wire. Then i made it into a shape and added shadow and inner glow.

I added some shadow on the body of the mouse by using the burn tool.

Then I took the gorgeous picture of the autumn leaves. Made them in the shape of a mouse mat and used the '3D Postcard from layer' tool (i think it's only available in CS4 extended)

Finaly I took the wood closeup again and used gaussian blur, Hue/Saturation, and levels to adjust it untill i liked it. And that's my final result. Hope you guys liked it!
