Then add the "hole" which wil show some flesh parts on the head. adjust the color a bit with Hue/Saturation (Ctrl-U) to give it a bit like a bloody/fleshy color

Ofcourse we then remove the texture which is over the "fleshwound" we do this by going into our texture layer, and hide it in the mask. then finally work the edges of the wound a bit with the burn tool, until u find it looking good

Then next you wil have to use the burn tool on parts of skin which are being overlayed by other parts of skin. (makes the spots darker)
For example the spots marked red.
ofcourse there are more spots which i think you wil know were to find!

Finally stretch up the skin a bit at places were the texture go off the skin, For example, the cheek!
