Started by painting out his shirt. Used a darker colr than the jacket as you are looking at the inside of it. I then added some noise and blurred it slightly.

I painted the wrist area and started cloning the original hand away. I also cloned the straps off the belt.

I masked out the skeleton source and skewed/distorted into position.

I masked the skeleton into position leaving the eyes visible as they were out of the light.

I added the skeleton hand.

I then made some lighting adjustments and added some shadowing to the bones.

I then copy merged the pirate (still keeping my old layers separate just in case) and masked him out and placed him on the new background.

Next I added a shadow (corrected after CMYK's advice) and also a ring in the sand where his foot is.

I used dodge and burn on the bones to correct some lighting issues.

I finished by using some color overlays to blend the images. I also darkened the image and allowed light directly to the pirate.
