first copy the layer. then select the eyes and press ctrl/cmd click. this will copy the eyes. next go back to the original layer that was copied. then use the clone stamp and patch tool to erase the eyes and mouth and nose.

then add the mouth. and use the erase tool at a soft brush. then go to image> adjustments> hue and saturation and set it so a color close to the skin

add the teeth. you will have to make several copies and flip them. then go to image> adjustments> hue and saturation and set the saturation to -61

add the teeth again and do the same as the last step
step 5 of 5
now take the eyes and move them down and use the erase tool and set it to a soft brush. then erase the eye brows and out side edge.
