Started by cutting the mouth out and transforming it into position.

Using a layer mask I masked out the mouth.

Played with the Color Balance on that layer to match the color of the mouth with the frog.

I cloned out the frog's nostrils as they were not required.

Again I copy/pasted the girl's face paying particular attention to the nose this time. I transformed the nose to suit the frog.

Using the layer mask again I masked out the nose.

Using Color Balance and Brightness & Contrast I color matched the nose.

Using the clone tool on a separate layer, I used the texture of the frog to go over the nose. I was careful to create the correct shape so that it didn't look flat.

I copy/pasted the frog's mouth so as to obtain the lower lip texture. I then warped this to the shape of the new lower lip.

Using the layer mask, this was also blended in.

To finish I sharpened the image, made some color adjustments and added a photo filter.
