I use one pill from source image and draw a shape of a caterpillar on a new page.

Then i give a shape to a pill as a shape of Caterpillar's body with the use of switch between free transform tool.

I took Green background on another layer and draw a shape of leaf and give some color and use Oil pastel brush as a eraser to erase a leaf for natural shape.

Paint the Background and apply noise.

Painted background layer and painted leaf Layer.

Give some Detail on a leaf and a little branch and put some highlight also.

Balance Hue Saturation and color.
