Used blur on the image and later used it again - I think it was radial blur...

Image of baby extracted from the original and placed on the background.

Cuddly pumkins added and duplicated a few times. The orientation of the eyes were changed on some so that they were all looking at the baby. This ensured that the highlights and shadows were in the right place.

Light lemon added to the background. Overlay and colour balance added to pull the image together...
