I use the "That just doesn't look right" method, so I start out rough and refine as I go along.
I started out with a very basic shape and a very basic outline.

Here I used dodge and burn for the features and painted in the mouth and teeth.

Smudging and a little more dodge and burn on the features. I painted in the iris, whites and reds of the eye.

Here, I carved down the mask, painted in the eyebrows and burned some eyelashes.

Once I finished with the mask, I started on the skull with this blob.

I didn't like the way the neon green was working out, so I used color balance to add more yellow and red to it, while still keeping the greenish tone. Then I burned in the teeth.

I burned some cracks on the skull and added a ribbon to the mask, using dodge and burn to shade it.

I decided that the skull was too bright, so I reduced the opacity of the skull, while leaving the eyes at 100%.
I painted in the body, dodging and burning for shading, then reduced the opacity on that, too.

I was originally planning on making the body match the mask, but I thought it looked better if it matched the skull, so I used color balance, adjusting the yellow, green and red.

I didn't want him to just be green, so I decided to have him peeling out of his skin.
First, I painted the skin, then added the flaps. I used dodge and burn to give the flaps texture.
