Reference photo used to design shapes which were made from the lemon. 'Coral' shapes were made with the brush tool, then highlighted and shadowed with dodge and burn tools. There are two layers of coral with fish in between.

Source photo is raised in resolution and sized to 10 by 25 which also stretches the pitcher and water splashes.

Here the lemon is being changed into the 'coral' by using the smudge tool, then eraser tool to finish off the edges. Highlights made with dodge tool. Scale tool is used to extend the coral up higher into the pitcher. Layer is duplicated so that fish can be placed between the two layers.

The 'coral' shapes are pretty well completed now with highlights added.

Fish is chopped using the magnetic lasso tool, then fine-tuning the edges with the eraser tool and blur tool. Fish color is changed as well as the colors on the pitcher using the hue/saturation tools.

Another fish is duplicated and placed above pitcher after rotating and flipping it vertically as if it is jumping right out of the pitcher. Shape is slightly changed using liquify warp tool to show it is twisting as it jumps. Some motion blur has also been added to this fish.

Some work was done to soften edges of the splashes, and a base of soft brown has been added under the pitcher with burn tool creating a shadow. The only think done to the rest of the background was cloning some of the background texture over some letters, and distractions which were there originally.
