step 1 of 5
Cut hands and brought into source image. Used transform to size.

Erased one hand and concentrated on the other. Used liquifer tool to move down the bottom of the eye. Transform hand a bit to suit. added some shadow

Same again with other hand. I had to paint in some white and try and create the bottom part of the eye
step 4 of 5
Added in some small red veins and the 'red' bit around the bottom. Used the dodge tool to try and create some highlight the there.

Messed about with some adjustment layers added a bit more shadow hear and there....
