I used a rough sketch as a ref.

In 3ds max started with a plane object. Then converted itto editable poly. Adjusted the vertices to basic shape. then added a symmetry modifier.

extruded edges and adjusted the vertices to sahpe the mask.

used bevel command to make the teeth.

I used boolean operation with some cylinders to make some holes just above the teeth.

i used multi-sub object material with some other standard materials for texturing.
