I took the background picture as is.

Then I took the picture of the feet and blacked out there area where the head is by selecting it then using the fill bucket thingy and then clone for the little bits that didn't go to black.

Then I selected what was left and cut it and pasted it in the top left corner of the background picture.

Then I took the picture of her holding the baby with the head showing towards the camera and made a second layer and made it gradient black and white.

Then I selected around the baby and hands (leaving out the wrists of the mom) and copied it and pasted it in the background picture and resized it.

Then here's the final product! But I did add another layer and do a surface blur and then lowered the opacity a bit.
