Used the liquefy filter and got this shape, was not planning this but the skull shape intrigued me.

Duplicate layer, go to variations and add more red 2x. Add canvas texture, erase head and figure. Finally duplicate main layer again and go to variations shadows and add red 2x again, erase selected areas.
