The basic shell was created using a sphere, a boolean cube extract and then a lot of poly modelling, I also applied a formula effector to the skin and inside mouth to give a ripple effect.

Added the teeth indicvidually which were created using a number of splines and loft nurbs.

The tongue started as a simple cube and then I used some matrix extrudes to create the interesting shapes.

Finally I added simple spheres for the eyes and a hyper nurbs over the whole thing, well except the teeth.

The chain was created by first making a single link and then duplicating along the spline drawn with a rotation applied.

The room scene was just a cube and then some platforms for the level were put in, these are ismply cylinders.

Added boxes and coins, simple objects just slightly altered for better detail.

Added a fractured cube to create a bunch of broken stone in the room.

This shows the lighting and the camera.
I used two lights, a green visible light that was used for the smoke effect thing and a spot light for shadows.
