First of all this rough design was sketched in with brush tool, using the grid to keep lines relatively straight. Having watched the 'Cake Boss' show many times, I tried to incorporate different layer shapes for added interest. Toward the end, the bottom layer will be made higher to better balance the overall design.

With dark blue background, the 2nd layer from top has color brushed in, and a design started on it w brush tool.

Whole image has been duplicated and moved to left and downward. By connecting with the back layer, a 3D look is created. Some use of perspective tool will be done here also to make layers look more dimensional.

Little roses are created using brush and smudge tools. These will be used in two different colors, joining them together to use as 'piping' along edges of cake layers. Distort tool will help to fit roses to side edges of cake to help perspective.

Many roses have been added along edges of layers, 2nd layer from top has had color brushed on it in a striped pattern, and a scroll design has been created on bottom layer.

Scroll design has been duplicated and, using grid, has been scaled to fit across the layer. Distort tool changes shape for side of layer.

Birds are created and detailed using brush tools and smudge tool, then dodge and burn tools used.

Birds are ready to add, so the branch is brushed in next. Textures are added with filter/glass/crystallize tool. Burn and dodge tools will finish off edges after birds are added.

Brush and smudge tools start shape of flower, then eraser and smudge tools further refine the shape. Fine brush tools are used to do the details of the petals and stamens.

Flowers are scaled to several different sizes, and some are rotated to make interesting groupings. After adding, burn tool is used under flowers, and inside them where they might be more shaded.

Blue background has had noise and difference clouds added, then a filter/render/omni light at about 75% intensity add a complementary light shape behind the cake to highlight it.
