use a low poly sphere and then squash it on 2 edges to make the ruby centre piece.

Made a river bed for the crown to sit in and positioned so you could see down the river with the crown partially submerged.

Added a water layed and another river base which was stretched on the y axis to make side of the river.

This is the shot for the rest of the image, placing the camera now means that everything else can be placed in a way that suits the final render.

The stones were pretty difficult because I has to create 4 different stones and then place them into a cloner, then used a randomiser on location and on scale and rotation. Then they were turned into objects and ridged bodies and allowed to fall on the riverbed as a collider, then removed the body and moved them down a bit.

Made 3 background mountains from landscapes and rotated and moved them with points differently.

To make the snow for these mountains I duplicated them and played with the high and low points so that the snow came thru from below.

This shot shows the lights and placements, I also added a sky and turned the 3 omni/spot lights to include caustics which worked with the water for the nice lights.
