I placed the image of the face and duplicated it and flipped it horizontally to make a full face.

I added the tree and grass image to the top of the head.

In this step I added the balloons and clouds image I found at stock.xchng.

Here I added the elephant and the giraffe.

In this step I added the birds using a bird brush.

Used a gradient here to create a rainbow.
