isolated highlights and shadows at several fuzziness levels using select->color range. Added color overlays to each layer in varying decreasing shades of red and blue and combined to get this result. (some of the image has been erased to preserve light or dark areas.)
step 2 of 3
isolated the cane from the original image and put into a new layer. enhanced the jewel at the top by duplicating the layer, adding some color, and adding a white glow behind it.
Also isolated the face and hands and put them into another layer. Adjusted the saturation, used dodge, etc to achieve the following.

cut the man out of the original image and placed a copy on top of the color overlay layer.
Added the shading on the glasses using gradients.
made minor adjustments to shading of shoes, shirt, hat, etc.
Added adjustments: two photo filters, vibrance, levels, and color balance.
The cane and the gradient on the glasses were excluded from the filters.
added a bit of a pattern to the white background for texture.
Done!
