i started making the background image.For this i use my 3D packages.
Starting with Sculptris and finalising and rendering in Carrara,wich leaves me with a
2D image for use in Photoshop,or in my case
Gimp2.8

Opened the image in Gimp

Imported the main image in a new layer,deleting everything except the face,rotated the face,and lowered the opacity.

Added an extra layer for the two red dots.
step 7 of 7
Exported the 3 layers together in a final jpeg image.
