Double click on the background layer in the Layers palette, when window pops up rename the layer or just hit enter.

In the Filter menu, select Artistic, then Sponge. When the window comes up alter settings for desired result.

Once again go to the Filter Menu, select Render, then Lens Filter and play with the settings till you achieve desired result.

Select the Text tool and click anywhere on the image. Before you start typing, pick desired font and font colour. Type in your text.

In the Edit Menu use the Transform tool to Rotate and Scale your text layer and move it where you want it.

Making sure that the text layer is selected in the Layers palette, select the fx tool at the bottom of the palette. Select Drop Shadow and adjust settings as needed.

While the fx window is still open, select the Bevel and Emboss and adjust settings.

The last thing I did was to make sure layer 0 was selected in the Layers palette, then went to the Image Menu, then Brightness/Contrast and adjusted the settings to my satisfaction.
