To model the screw I used a cylinder shape with 2 height sections. The center edge was expanded and twisted to make a section of thread. This new geometry was cloned several times and stacked to form the length of the threads. The screw head is a cube which has a turbosmooth modifier with 3 iterations. Certain polygons were selected and a bevel mod added in order to create the screwdriver slot.
