I painted over the source image on a new layer and tried to define the shoulder muscles and general shapes. I used a round soft brush. I used google image to look at references for the bones placement and things like that.

I duplicated the eyes and placed them on a new layer to change the direction they are looking at. I painted a thumb to the troll's left hand. I added texture to the troll's skin by duplicating the texture found on the troll's face and left forearm and painting it, then played around with the blend modes and opacity.

I extended the canvas and copied some rocks of the source image that were painted over in the 1st step.

I added the tip of a sword. I looked at some references with google images, but I already had an idea of what I wanted it to look like. I used polygonal lasso tool and gradient tool, then I painted some textures with a round brush and played around with the blend modes and opacity until I was satisfied.

At some point I started to paint a background but wasn't satisfied with the direction it was going so I decided not to include it. Also I like the original blurry background.

I painted the shape of the cape, then I used the free transform tools to give it a shape I liked. I painted the golden thread around the edge with a round and soft brush. I added the shadows of the cape with a very soft round brush.
