It's a pretty fine mess, but I kind of miss the rule of third in this one. Did you try using a tripod for making the photo? Good luck!
The Jug is is what appears to draw the eye the most so it is placed in the right lower intersection. I thought that it might work for this comp because the lines of the couch and chair lead right to it. Lol.... its probably not as apparent as I thought it was!

Still you have a way of showing humanity in all its forms,you really are a caring person with a sense of whats right and sometimes showing this sort of work can be disturbing and want to look away,sad,sad picture this and hope she got help,thanks for sharing this even if its good viewing.
Thank you. These kinds of scenes I have seen all my photographic life. Even when I was studying photography I would alway gravitate to those less fortunate and shoot them, perhaps because I see myself in them.
I can feel the connection between the phobia and your image. Sad but as a photographer you made the connection and I take it as a voice, as a reminder of keep taking care of every living being. Thank you for that
