The black and white does this image no favors. It looks highly HDR, emphasizing every subtle skin tone variation, which detracts from wrinkles, and overly accentuates a very mottled skin instead.
Thank you MossyB for your comment, though I do not agree with you. I have deliberately used this effect as much to the image to be make it harder. I read an article of a photographer who is editing all his streetphotography pictures in this way to the increase the drama. I liked that style...
The strong contrast there shows all wrinkles, the 2 scars on the cheek and blemishes.
I think that's a matter of taste

I like the gritty processing on this image. I personally think it lends itself well to street photography. I think you have achieve the style well.
