Well its definately on theme, thats a really bad smile. Ok, so where are the sources? The ones for the teeth, lips, man?? The one listed for the cig - check with a mod, I am not sure its a legal image.. Nice idea. GL.

I'm not sure having colour lips on a monotone image is working...or the text on the image...but it's poignant and gets the message across...

You know he really isn't smiling.. since you are pretty skilled with the liquify.. why not patiently pull the corners of his mouth up so it turns into a smile.. just don't lose your original image in case it doesn't work for you..
