I use the child's picture taken from http://www.123rf.com/photo_4346902.html in the free image section. Then I cut the source image and blend it to the child's picture. Liquify a little bit to fit the eye. And add the flag using brush. (5 years and 3770 days ago)

The use of a displacement map around the eye circle would have improved the result. Nice effort anyway.
