The ring on the nose, i painter it [default brush],and also the smoke near the mouth done using smudge tool [default brush], but it doesnt look like smoke :p i used curves to adjust the brightness. andblood stain is done with brush [default brush] and the thing like wood in the bg painted it [default brush] (5 years and 2557 days ago)
